Does this extend longer than 18 inches?

No. The MKA-18 is a Fixed Length handle. It stays at 18 inches. If you need adjustable length, you should look at the MKA-45 (Telescoping 17"-25") or MKA-44 (Telescoping 24"-40").

Will this fit my Mercury 5HP outboard?

In most cases, yes. The clamp is designed to fit handles up to 1-3/4" in diameter. Most small outboard tillers fall within this range. It is a very popular, inexpensive way to extend a gas engine tiller.

Is it hard to install?

Not at all. It slides over your motor's existing handle and uses a simple wing-nut clamp to tighten down. You can install it or remove it in about 30 seconds without any tools.
